Additional helpful information for your appointment:
- List of all medications, both prescription and over-the-counter
- List of all drug allergies
- Previous allergy testing records
- Reports of X-rays and CT/MRIs
As a courtesy to our patients, many of whom are sensitive to odors, we kindly ask that you refrain from wearing perfumes and/or fragrances to your visit.
